A 40-year-old roofing contractor from Rockland County, NY, with a history of safety violations and two employee deaths, was charged with knowingly and willfully violating federal safety regulations, including failing to protect his employees from fall hazards, resulting in injuries and deaths. 

Las Vegas fire crews responded to a fire on the roof of the Fontainebleau Hotel property on the Las Vegas Strip, which occurred on July 22, with no injuries reported; investigators are determining the cause, and the 67-story hotel, under construction since 2007, has changed ownership multiple times. 

A single-engine plane carrying three people crashed into the roof of a Georgetown, Texas, home on Sunday. The three passengers aboard the aircraft were rescued at the scene and taken to a local hospital with non-life-threatening injuries. The home was unoccupied at the time of the accident. 

A roof collapse at a middle school gymnasium in China's northwest province resulted in 11 fatalities, with construction work at the school being identified as the likely cause due to materials being stored on the roof, according to multiple media outlets.
